Any type of groin strain can lead to these symptoms. Other possibilies would be a hernia, or less likely, an infectious cause - such as prostatitis or epididymitis.
A scrotal ultrasound can be considered to evaluate the testicular anatomy. A hernia can be checked on a physical exam, or a pelvic CT scan or ultrasound.
Infection can be evaluated via a urinalysis and a digital rectal exam to evaluate the prostate.
