UTI? Prostatitis? STD?

Hi,

I am a 23 year old male I am currently experiencing severe pain when urinating and straight after ejaculation. I have in the last week just been tested for the norm... chlamyidia, ghoneria, HIV etc and thay all came back negative.

My 1st symptom was a dull aching pain in my testicles (hence why I wen to get tested for any STD's) then after taking doxycicline for 3 days I developed the pain during urination then a further day I developed the pain after ejaculation... stronger after the ejactulation. I went back to the GUM clinic and they gave me water infection tablets, not knowing if I had a water infection or anything as I don't have any change in timing for the loo or how many times.

Can anyone shed some light on this? and what I should do next?

The good old NHS (UK) strike again.

Got all three scans today, now I have to wait so they can be sent to the doctor that consulted me and then the to my GP so then he can pass them onto me! What a load of....  

He did take quite a few pictures of my left testicle and right kidney, none of my left kidney or right testicle... hmmm..

Hi Smitha,

Yes past history of a kidney stone, my father had one when he was around 40 of which had to be laser blasted then passed through urination. However no history of cancer.

I will update next Thursday :)

Hi Colin,

Do you have any family history of kidney stones or cancer?
Any past history of kidney stones?

It is possible that a small stone passing through the urethra could have injured the lumen and caused the slight bleeding.

However, its best to have the investigations done in view of the lump.

Guess you have no option but to wait until your appointment :-)

Meanwhile, you can also increase your intake of Vitamin C as it has been known to mediate healing and increases mucosal integrity against injury and infections.

Update - my scans are next Thursday. I asked for earlier for thats the earliest they said I could have :(

Well all I can do is wait for my appointment?

The blood was very slight, just a spot or two but as previsouly stated I was just sitting in front of the computer when I notice it.

I have a symptom that I have not yet talked about, after ejactualtion I get a red smere accoss the tip of my penis where the semin has been, this goes away after about half an hour to an hour... however I have no pain during or after urination or ejaculating now just the red smere.

Hi Colin,

I am sorry to say that it doesn't appear to be a routine problem anymore.
You will need to pursue this a little more persistently with the hospital.

Even if the bleeding was slight, in the absence of any trauma, its appearance without passing urine is worrisome.

Please have the hospital run blood tests to evaluate your platelets and coagulation profile at the earliest.

The lump also needs to be examined through an ultrasound ASAP.

Please do not wait to have this checked.

Update - I went to hospital today as there was blood coming out the end of my urethra, without urinating or even moving.

I am goin in next week for an ultrasound on my kidney, bladder and testicles as I told them about the lump.

Fingers crossed.

I was told to finish the prescribed antibiotics.

Stress - no not more than normal, none at all if am being honest.

I will intake more yoghurt and maybe buy some pro-biotic drinks.

After consulting with my GP about the lump (when proscribing Penicillin) he said becasue I had been checked for a lump in the same region before I won't need to get checked again and is unlikley it wont be anything. Nor did he examine me in any way other than to look at my throat, this is after I told him I was still getting pains in my testicles... the lump is still there.

Hi,

Any lump on the testes will need to receive investigative attention.

You will need to have it examined and have an ultrasound done for it, no matter how harmless it appears.

Hi,

You will need to check if you need to continue the doxycycline.
It would be good if you can find out if a culture sensitivity was done for your urine and what antibiotic the organism detected was sensitive to.

Have you been going through any increased stress of late that could have adversely affected your immunity?

Penicillin was prescribed for the throat infection because some antibiotics are known to have better perfusion for some tissues, such as penicillin or azithromycin for throat infections.

However, it is not an ideal situation to be on so many antibiotics at once as they tend to eliminate the normal organisms in the body predisposing to gastrointestinal disturbances.

Do increase your intake of yoghurt to replace the "friendly" organisms.

I have been back to the doctors again! And am now on Penicillin!

3 antibiotics at the same time, is that safe???

Just as an update from the above, I have now developed a throat infection. There are white puss build ups on the left side of my throat. Not having the bestest of times at the minute! Most throat infections are viruses right? so the antibiotics I am on will not have any affect?

Hi,

I started the docxycicline last Wednesday and the ciprofloxacin on Tuesday this week.

However the dull aching pain is still hovering about and I have now found a lump on the front of my left testicle... not sore to touch but hard and kind of oval in shape... it is smaller than a pea.

A year gone december I had to go for an ultrasound for testicular cancer however they didn't find anything.

Should I contact my doctor again or could this be epididymitis, although the lump is at the front of my testicle not at the back.

Hi,

If you have noticed a decrease in the pain symptoms, it could mean that the infection is resolving.

How long have you been on the ciprofloxacin?
Ciprofloxacin is specifically prescribed for infections of the urinary tract due to its mechanism of action.

Do continue the other measures, such as adequate hydration and avoidance of caffeine and alcohol.

Hi,

I am now on Ciprofloxacin and Doxycycline.

However now when I urinate there is not very much pain during but at the end and afterwards, does this mean it is helping?

Hi,

It is possible that the organism causing the infection is resistant to doxycycline (since your symptoms have continued despite being on doxycycline for 3 days.

It would be best to go back to your doctor and report that the antibiotic has not helped.

You will need to take the antibiotic for the prescribed duration in the interim.
Also your urine culture report (if done) would be available now and that would really help decide what antibiotic is best for you.

Refrain from too frequent sexual activity as it can further irritate the prostate and worsen your symptoms.

Thanks for the prompt reply.

I have had no further tests done other than GUM clinic standard procedure... blood and urine was taken... I was told it may have been epididmitis caused by a water infection.

I have the antibiotics until Wednesday next week do you think I should wait until then to do anything further.

The pain after ejactulation last night was nearly unbearable.

Hi,

Have you had a urinalysis done? If yes, were there any organisms in it?
If organisms were present, has a culture sensitivity been done?

Even if the urinalysis came back negative it is possible that you could be having an inflammation of the prostate which may or may not be secondary to a bacterial infection.

It is possible that the symptoms you have described are a result of prostate inflammation.

You will need to have the urinalysis repeated, probably a prostate massage test also.

If any organism has been detected and cultures you will put on antibiotics that the organism is sensitive to.

Meanwhile,

Follow up on the same with your urologist.
Drink fluids in excess of 2000 ml per day
Increase intake of citrus juices and ensure vitamin C intake is over 1000 mg per day.
Sit in hot water for 10-15 min twice or thrice daily to relieve pain symptoms.
Reduce sexual activity to about 3-4 times per week.
Avoid, caffeine, alcohol, and spicy foods.

Do keep us posted on your doubts and progress.
