Urethritis + sore balls + sore prostate + sore inguinal nodes,

10 weeks ago or so I started getting sore groin nodes - right ones are far more painful than left, vetical more painful than horizontal. Non hemolysed trace of blood in urine at onset now gone. Pain subsided a bit then I realised my prostate was sore and occasionally my testicles. I was getting chills and fever in the evening. My bedclothes are drenched in the morning.

Prostate pain has died down a bit and now my testicles are nearly constantly sore (right more than left). The bottom of my back is painful too.At night more than in the dyatime I'm getting chills and rushes around lower abdomen, lower back and thighs  . I'm not taking any ibuprofen as my stomach is already sore and they disagre with me. Also last two weeks severe gustatory sweating. I was on Trimethoprim for a week and no response. It cleared a chest infection for a fortnight though.

I am breaking wind a lot as well (so maybe all this is a GI problem)

Back in January I had generalised lymph pain, arms, legs, groin, neck, head, chest ...everywhere.

CBC in March , August OK (no numbers Rxed). LFT KFT OK August

OK it would be simpler to say my pelvic area is sore.
I'm very close to getting fired as pain is keeping me awake at night.

I thought this was a bacterial infection (Chlamydia which was treated 7 Jan) re-emerging at first as symptoms were similar but that and all STIs inc HIV have subsequently tested (4th gen) negative (to 8 mths).
My GP doesn;t appear interested.

Got my Full Blood Count results from my GP today. They said before everything is nornal and within range, however the Basophil and Eosinophil are marked on the results sheet as Abnormal. My medicine books say they can be signs of parasitic or bacterial infection. The red blood count also suggests I'm anaemic (though I could have measured that in my busy sleep schedule and the number of layers I have on to keep warm brrrr)

Last 3.5 months I've been experienceing fever (usually 38.1, one occasion 38,6) and chills, which seem to be in time sequence with pain in my pelvic area. Fever and chills seem to have settled to an occasional level now late morning , late afternoon and evening so possibly in relation to my digestive cycle. It's difficult to place where the pain is.. was the perineum, have had proximal urethritis and also sore testes. On a few occasions I've had night sweats which have drenched my pillow. I'm letting off wind a lot, had swollen groin nodes diagnosed by an A&E (ER) Doctor when I had chest pains and a numb arm (ECG OK, no MI enzymes tested for).

I've been passing blood (sometimes fresh, sometimes black) and mucus in my stools for some time now. Stools are nodular to start so obviously some inflammation in my colon.
My GP has given me a referral to a GI surgeon (Proctologist) for follow up as I hae previously had piles.
He will send away urine to the path labs but I was unable to provide a sample this morning. Looks like I am getting somewhere now though.

Many thanks,

Dr Mathews,
Many thanks for your detailed reply.

The pains are reducing a bit in frequency. The fever and chills remain.

I'm drinking enough, mainly tea (roobios, green, black and herbal)
I haven't had sex since my exposure on 4th December.

I think I will give it two more weeks to see if symptoms disappear completely then see my GP again, though I think he thinks I'm making all these symptoms up. He also inferred anxiety (about HIV exposure) could be playing a part in this. I will take along a sample as you indicate, would first pass one from first void in the morning be appropriate?

MEDICAL PROFESSIONAL
Hi,

It is possible that you have prostatitis. This is caused by an inflammation and infection of the prostate gland. The infective organisms may be located only within the prostate and/or kidney and come into the urethra during ejaculation. If they are not flushed away by urine, they would continue to be present in the urethra and try to grow there causing irritation and pain. As the nerve supply to the prostate and the penis and testes are by the same main nerve, inflammation of the prostate gland can also cause pain and soreness in the testes and penis.

You may need to have another urinalysis done, preferably from a sample collected just after ejaculation. A prostate fluid analysis can also be done. This is to identify the organism causing the infection and may be followed by a culture sensitivity test to determine which antibiotic the organism is sensitive to. A prostate ultrasound (done rectally) will give an idea of the state of the gland and whether it is enlarged.

Antibiotic therapy is prescribed for long durations in prostate infections in view of the poor permeability of the prostate gland to antibiotics. The course will be for about 15-21 days and may even be for a few weeks. If you had been infected with chlamydia and the infection had lodged in your prostate, it is quite possible that it was not eradicated completely.

Ensure that you keep your fluid intake over 2000 ml per day and avoid irritants, such as caffeine, alcohol, and spicy foods. Also avoid sitting for long periods of time or on surfaces that are not well padded.

If you do indulge in intercourse during the period of treatment, ensure that you practise safe sex (condoms) so as to not infect your partner.

Do keep us posted on your doubts and progress.
