Urethritis or prostatitis?

Hello! Im a 20 years old male. Not sexually active.

Ive been facing urinary difficulties for more than 2 years now.
Generally there is no pain or burning when I urinate. Just dribbling, difficulty in starting stream and urinary retention. No discharge. The symptoms exasperate or dubside from time to time. First the GP thought it was a bladder infection and prescribed antibiotics. No relief.

Went to a urologist and he perfomed some tests

Urinalysis - normal
Ultrasound - normal but post void residual volume of 80mL.

He prescribed cefixime which also didnt work.
Then had a reyrograde urethrogram to rule out stricture. And finally cystoscope showed inflammation in the urethra which he assured would go away with antibiotics. The problem didnt disappear completely but I got better.
Stopped taking mddications for more than a year and it started all over again. Im also experiencing burning amd pain in urethra when not urinating. My bowel movements are also affected.

How can it be urethritis when there is no infection or std?

Why havent the doctors considered the possibility of prostatitis?
