Urge to urniate all day and especially after orgasm

First, I feel I have to urinate all the time like every 30 minutes. The urge to urinate is from the uretha, not the bladder, more so in the morning than at night. I usually have to get up once per night to pee but two or three times lately and not a lot of pressure. My uretha is moderately painful and burns a little bit but feels better during and after urination. It is worse when I am sitting down, better when I am walking around. It just feels like there is something inside my uretha burning the tissue. Also, occassionally right after and often about an hour after an orgasm I feel the constant urge to urinate even though I have emptied my bladder. That is extremely uncomfortable I want to urinate to relieve the pain but I have nothing to pass. Other mild symptoms include mild abdominal and low back pain, some muscle aches in arms and legs, sharp pains behind my pelvic bone and groin, and my penis seems to be very sensitive to rubbing against my clothing. Last week I had a mild headache all day and a little weak I had a full STD test (all negative) because of similar symptoms in July. I have been with the same partner since May and I was married 15 years with no problems before that.

The negative STD test would rule out the more common causes.  I would also send the urine off for analysis and culture to further exclude infection.

Any prostate enlargement can also lead to urinary frequency.  A digital rectal exam to examine this, as well as a transrectal ultrasound can be done.  Massaging the prostate and sending any fluid off for analysis and culture can be considered to exclude prostate infection.

If the symptoms continue, I would consider a cystoscopy to evaluate the lower GU tract.  More specialized testing like urodynamic studies can evaluate for any functional deficits.

These options can be discussed with your urologist.
