Urinary Incontinence

I am a 38 years old healthy male but have been having some incontinence issues since the last two weeks.  The main symptoms are (1) weak urinary stream (2) dribble at the end (3) a bit frequent urination during the day but not at night (4) good stream when it appears that bladder is full (5) appears that urine is stuck in the urethera (penis area) and about to leak out, and this requires me to rush to restroom but only a small amount comes out, (6) feeling of leaking out happens after apparently a good void, (7) when i come back from a good void and sit on chair, I feel that urine is going to leak out, (8) dry blood in semen 2 years ago but not any visible dry blood in semen any more, (9) microscopic blood in urine 2 years ago but none in two tests in last 10 days, (10) very small kidney stone in one kidney, (11) no burning/pain during urination, (12) no sexual diseases, (13) blood test is normal with normal PSA, (14) PCP checked prostate and says "boggy" prostate, (15) slight pain on left lower abdomen since last 3 days, (16) bit of itching at some places in the body (primarily feet area) since last 2 weeks. (17) urine culture ok.  Any tips or ideas will be very welcome.  Thanks in advance.
