Dear Richard,
Before BCG treatment was available, CIS of bladder was treated by cystectomy (taking the whole bladder out). This was due to the aggressive nature of CIS in the bladder. Since BCG, however, the need for cystectomy has decreased by 75%.
Several independent studies have shown the tumor free response to BCG therapy to persist for more than 5 years. The overall evaluation showed 72% of the patients to be complete responders. Of this group 64% remained tumor free for more than 5 years. Most recurrences tend to occur in the first 5 yrs. Of the patients followed up for 10 yrs. only 36% have remained free of progression and recurrence. Another study showed a 21% recurrence rate in successfully treated patients during the 2-10 yr. follow up. Lifelong follow up is clearly indicated in all patients who have been successfully treated with BCG.
