a unique urology problem????

I am 47. I go to bed at around midnight. fall asleep. depending on what I ate 4-6 hours later I wake up with pressure. I have a very hard erection. I feel like passing gas and urinating. I cant pass gas and I cant reverse my erection. it as a lock. the only thing I can do is to  urinate through my erection which dribbles a small amount. then immediately erection subsides. I pass gas and the system relaxes. I go back to sleep. a milder version of the same repeats in few hours. I wake up. urinate. it feels restrictive. then I do the bowel movement. it comes out a small piece.  a bit more urination and pass gas then a substantial bowel movement. tightness in the area lingers until noon. in the afternoon I urinate totally unrestricted, in quantity and velocity, everything behaves normal. I go to bed at night again. whole episode repeats. swaps/ urine test were negative on bacteria/std/ yeast. regardless I used antibiotics, no results. in the past when I had similar symptoms antibiotics made the problem go away immediately. not this time. I seem to have a  recurring crampy feeling an inch above the base of my penis , in front of my abdomen. symptoms are here for the past 3 months. some days are better then others. however sex makes a huge difference. if I have intercourse before going to bed (with or without condom), the intensity of the symptoms multiply. cramps get sharper. general feeling of inflammation increases and stay longer in the area, for an entire day or two afterwards, slowly subsiding. a certain level of discomfort starts during sex. with erection on, I get the need to go urinate in the middle of the action few times, if I don't it also gets crampy. there is a strange interaction between urination, ejaculation, gas and bowel movement. they seem to be all locked up as a result of an inflammation.any ideas?

Tough to say without evaluation.  Problems with the penile vasculature can lead the erectile symptoms.  Regarding the crampy feeling, you may want to consider prostatitis, where chronic infections can lead to a dull ache.

An ultrasound of the penis can be considered to evaluate the vascular function.  You may also want to consider a digital rectal exam, as well as a urinalysis, which can help evaluate for prostatitis.  Tests for STDs such as gonorrhea and chlamydia should also be considered.

I would consider a urology referral for evaluation and discussion of these options.

My sister is 15 years old and lately she feels like she has to go to the bathroom very often.
