abnormal penis pain (internal)

Hello,

Since about a year ago, I have been experiencing significant pain in the inside of the shaft of the penis. The pain occurs in both flaccid and erect states, although in the flaccid state it comes and goes, and seems to recede after several days, but in the erect state it is constant (feels as if the pressure is too high). After an ejaculation, the pain seems to return and the cycle restarts. The pain in the flaccid state varies between mostly burning and sharp twinges. All the pain is "inside" (underneath the skin) the penis and feels like it is towards the top of the shaft of the penis. The pleasure is much reduced in sexual activity, and the pain makes sexual activity mostly unenjoyable. But otherwise all basic components of sexual activity are there.

I might have a clue what's causing this. About a year ago I went a bit crazy with sexual activity and somehow managed to have 4-5 ejaculations in about an hour period, plus one or two earlier in the day. I tried again a last time but it actually hurt, so did not continue. I'm pretty sure this is when my troubles began.

The penis looks and feels normal, except that there could be a very slight bulge near the top during erection, and i do notice also near the top I feel slight ridges underneath the skin in a flaccid state, although I can't state if any of this was or was not there a year ago.

So generally I am wondering if this is some sort of disease or if I did the damage to the penis. Is the damage permanent? Since it hasn't healed in a year, will it ever? Or will surgery be necessary. If it is scar tissue or chronic inflammation, is it in any way healable?

Thanks very much for any and all your help

MEDICAL PROFESSIONAL
You can consider an evaluation for infection such as an STD or UTI.  A urine test or urethral swab can be done to evaluate for this.

Imaging the penis for any trauma, scar tissue, or hematoma can also be considered.  An ultrasound would be a reasonable initial test.

Treatment and prognosis would depend on what is causing the symptoms.

These options can be discussed with your personal physician, or in conjunction with a urology referral.

Followup with your personal physician is essential.
