can't stay hard during sex

My wife and I have been together for 14 years and previously we had a good sexual relationship. For about the past 5 years sex has been nonexistent. She wants to have sex but I have had problems with keeping an erection during intercourse. If she gives me head I am able to maintain an erection and have multiple orgasms. It has gotten to the point where I don't even try anymore and I know she is frustrated. We might have had sex twice over the past year and maybe a dozen times over the past 5 years. I am 43 and in ok shape, I have a history of testicular cancer but have been in remission for 14 years. In the past when I have had problems she would get upset and cry because she thought it was her fault and I am kind of afraid to upset her. I love my wife very much and want to be with her, any suggestions?
