chronic left testicle pain

by urgent282, Jul 28, 2009 08:11PM
I am 26. When I was around a year old I had pediatric herina surgery which I believe is the cause of my problem today.Through the years I have always had discomfort but just lived with it. About 3 years ago I decided to seek help Going from doctor to doctor (M.D.'s, Urologists,General surgeons) and having test after test (CT, urtrasound) to find nothing to cure my pain. My left testicle is 1cm smaller than my right and I have pressure, discomfort, and feel as if things are out of place. Most doctors I have seen told me to live with it. I CAN'T! The doctor I am currently seeing seems to be making sense. He said the herina surgery I had 23 years ago restricted the blood flow to my left testicle, that is why the left testicle is 1cm smaller than the right. It is like wrapping a string around your finger, He said the damage is already done that there is nothing he can do but to remove the testicle. The pain is getting to the point that I can't stand it. I am on strong pain medicine that is not helping. I don't know what to do or where to turn is there any advice you can give me. PLEASE HELP!!!!!!

Hi
Thanks for writing to the forum!
Yes, in your case poor blood supply to testicle could be the cause of both pain and smaller testicle. The pain could also be due to pudendal neuralgia. “Prostatitis-like urogenital pain and voiding and sexual dysfunction  are the hallmark of pudendal neuropathy.” Refer: http://www.chronicprostatitis.com/pne.html
Pain in testicle could be due to infection, like prostatitis, epididymitis or orchitis. Get a culture sensitivity test for both semen and urine done. A prostatic massage and evaluation should also be done. Also it could be due to referred pain of kidney stones. An ultrasound of the lower abdomen will be able to rule out kidney stones. It could be due to cancer of testis. It could be due to varicocele or spermatocele. An ultrasound of the testicles will diagnose cancer, varicocele and spermatocele.
It is difficult to comment beyond this at this stage. Please discuss these possibilities with your urologist.
Hope this helps. Please let me know if there is any thing else and do keep me posted. Take care!

this is urgent282 I went to see yet another doctor (director of urology) He said it has to do with my nerve receptors and put me on Venafaxine. I don't know what to think anymore. Any suggestions?

The point is what you want or need.

I have been there and the pain got so bad that injections of pain meds only took the edge off. I later had to have the testicle removed. The conclusion was that former inguinal surgery had damaged nerves/ blood supplies. Surgery is not the only option though and I advise men against taking the final option if there are other choices. Nerve blocks being a good one. Work with your doctor to moderate your pain. Living life with one testicle is of course not so much an issue but it took me over a year to get used to it.

Hi
Well, nerve receptors theory is more in line with pudendal nerve compression. At least the diagnosis is now pointing towards a nerve pathology. Consult a neurologist and see if he can throw some light. If your sperm count is normal and the semen test is normal then if the pain is manageable with the new medication, you should try it. Alternatively you could learn some pelvic relaxation exercises after consulting a chiropractor.
Hope this helps. Please let me know if there is any thing else and do keep me posted. Take care!
