Yes, in your case poor blood supply to testicle could be the cause of both pain and smaller testicle. The pain could also be due to pudendal neuralgia. “Prostatitis-like urogenital pain and voiding and sexual dysfunction are the hallmark of pudendal neuropathy.” Refer: http://www.chronicprostatitis.com/pne.html
Pain in testicle could be due to infection, like prostatitis, epididymitis or orchitis. Get a culture sensitivity test for both semen and urine done. A prostatic massage and evaluation should also be done. Also it could be due to referred pain of kidney stones. An ultrasound of the lower abdomen will be able to rule out kidney stones. It could be due to cancer of testis. It could be due to varicocele or spermatocele. An ultrasound of the testicles will diagnose cancer, varicocele and spermatocele.
It is difficult to comment beyond this at this stage. Please discuss these possibilities with your urologist.

this is urgent282 I went to see yet another doctor (director of urology) He said it has to do with my nerve receptors and put me on Venafaxine. I don't know what to think anymore. Any suggestions?
I have been there and the pain got so bad that injections of pain meds only took the edge off. I later had to have the testicle removed. The conclusion was that former inguinal surgery had damaged nerves/ blood supplies. Surgery is not the only option though and I advise men against taking the final option if there are other choices. Nerve blocks being a good one. Work with your doctor to moderate your pain. Living life with one testicle is of course not so much an issue but it took me over a year to get used to it.

Well, nerve receptors theory is more in line with pudendal nerve compression. At least the diagnosis is now pointing towards a nerve pathology. Consult a neurologist and see if he can throw some light. If your sperm count is normal and the semen test is normal then if the pain is manageable with the new medication, you should try it. Alternatively you could learn some pelvic relaxation exercises after consulting a chiropractor.

this is urgent282 I have been on Venafaxine for about 2 1/2 weeks and it is not helping my pain. I also am taking Aleve (4) at once like the doctor said. I don't know what's next?
this is urgent282 I made an appointment to see a medical doctor. My left testiclar pain did not get any better. Venafaxine that I am taking is not helping. 12 doctors and nobody can find the cause of my pain. Where can I turn next? Am I ever going to find the right doctor to help me so I can live a life without pain.
