constant pain and lump in testicle, no sex drive

My boyfriend has been to his doctor complaining of pain and a lump in his testicle for years, finally she gave him a referral to a specialist.  The specialist looked at his ultrasound and said that there was nothing there. My boyfriend made him feel the lump to prove to him there was a problem. The urologist then said it was just a hydrocele and nothing could be done. My boyfriend is intense amounts of pain, apparently hydroceles are not supposed to cause pain. His family doctor won't give him any more referrals. We saw another doctor who said that he thought it was definitely not a hydrocele but he couldn't give us a referral because he wasn't our family doctor. I HAVE FELT THE LUMP, IT IS THERE.  This is becoming increasingly frustrating.  A major thing I've noticed is that my boyfriend has NO sex drive at all. He is 21 and only masturbates a couple times a month, we only have sex 1-2 times a month and whenever i try to initialize it he turns me down.  We both live on our own (in seperate apartments) so I really don't see any reason for this. Maybe these things could be related?

At first I was worried, and now I'm just starting to get really angry at the medical community and the way he's been treated.
