Cancer is a disease that grows very fast. This is because the cells affected by cancer become abnormal and start drawing all the nutrition away from the normal cells.
This results in marked weight loss and manifestation of symptoms specific to the system affected.
As you have had this enlargement issue for over 5 years, it is highly unlikely to be any sort of cancer.

Hi thx for your response it helped me relax a bit. I know i should go to the doctor but im waiting to get access. Since i had my right testicle larger than my left for more than five years would i be really sick if i had cancer? I had this problem 1 year ago and half ago before and the doctor felt my testicle and said i was okay . no lumps so hopefully i will be okay thx for your help.

It is not stupid to say you do not feel a lump. It is possible that you just have some collection of fluid or other cellular debris and they can be contributing to the enlargement.
If your doctor did not feel anything either, then it is possible that there is no problem, but you can always have an ultrasound done for confirmation.
Some individuals can have one testis slightly larger than the other and usually one testicle hangs lower than the other.

If you had a hydrocele and were operated for the same sometime back, it is possible that the hydrocele may be coming back. This will need to be evaluated by a urologist and will need to be confirmed on an ultrasound examination.
However, it is very unlikely that the pain in the testes and your leg are due to cancer. The usual presentation of cancer in the testes in males is a painless lump and there may also be blood in semen.
Other reasons for the pain include an inflammation of the epididymis or the prostate.
