extreme pain in testis

Hello, I am a male 23 yrs old and have extreme testicular pain and what feels like swelling in both testicles at the top and bottom for 12 weeks now.  
I have had 3 urologists see me. all 3 did ultrasounds of my testicles.  All came back clear.  I have had complete blood work urine work, all clear.  
I was diagnosed by the 1st with epidydimitis and put on 6 weeks of levaquin and an anti inflammatory with no help.  The second diagnosed prostatitis and continued my antibiotics and antiinflamms, no luck.  the 3rd diagnosed a muscle problem. put me on flexeril and demerol.  These drugs have helped my pain a little.  
I also have lower back pain and shooting pain in my legs.  I have had an mri of my pelvis, brain, sacrum and lumbar all were negative.  I have had xrays of my tailbone, neck and back. the xray of my back did show a narrowing between the 4th and 5th Lumbar.
I have been receiving chiropractic manipulation for 5 weeks now with some temporary relief. I have been in bed for 12 weeks now.  I have gotten better 4 times. each time lasting 2 -4 days.  
I have figured out that when I stand up to go to the bathroom, my testicles "swell" and hurt.  I can alleviate the pain and swelling somewhat by lying back down or squatting.  I also notice that when my testicles are pulled up(when I am cold or my feet are cold) I have no pain or swelling when I get up and walk.
At this point I am considering pain mangement.  Has anyone heard of this intermiitant swelling and pain coming and going like mine.
I am confined to lying down because of fear of the pain starting again.  I just graduated College and want my life back.   Please help?

You state you have been to 3 urologist as well as multiple MRIs of the pelvis, brain, sacrum and lumbar spine.  You have been treated for epididymitis and prostatitis without success.  

That is a very thorough evaluation - the only other urologic evaluation would be a transrectal ultrasound to image to prostate.  If the urology route is completely not revealing, you may want to consider referral to either a neurologist or physiatrist (a rehabilitation specialist).  It is possible that the pain may be referred from the narrowing of the back, or may be due to a nerve problem.  You may want to consider an EMG by a neurologist for further evaluation.

Many physiatrists are also pain management specialists who can suggest a chronic pain relief regimen.
