Testicular swelling should be further investigated. Causes like infection, cysts, hydroceles, varicoceles can all be present.
The best test would be a scrotal ultrasound to investigate for each of the causes listed above. I would also test the urine for an infection, as well as be tested for STDs such as gonorrhea or chlamydia.
Discussion of these tests should be done with your personal physician. You can also consider a referral to a urologist for further discussion.
