intermitting swelling on right testicle

i have an intermitting swelling on the bottom of my right testicle and both testicles become hard when the swelling appears and i get some discomfort in my groin area,it happens on and off throughout the day but i notice it more when i have a hot bath or have an erection and when i ejaculate the sperm is very slow in coming out, when i have this swelling my right testicle seems to point straight out,after maybe half an hour they go back to normal,this has been happening for over a year,can you tell me what is causing it.

Testicular swelling should be further investigated. Causes like infection, cysts, hydroceles, varicoceles can all be present.
The best test would be a scrotal ultrasound to investigate for each of the causes listed above. I would also test the urine for an infection, as well as be tested for STDs such as gonorrhea or chlamydia.
Discussion of these tests should be done with your personal physician. You can also consider a referral to a urologist for further discussion.
Followup with your personal physician is essential.
