: Am 53yrs.old and not in pretty good health. Have had a slight diabetis problem that I've been able to keep under control.  For the last couple of yrs. I have been have a problem of keeping an erection. When I am able to keep one long enough to be able to make pentration I have almost instent orgasim.  This problem has become worse as the time goes on.  Have noticed that when I do get a partial erection any more that I have a bad curvature to the side.  This has become quite bad.  Always did have just a slight curve but nothing like this.  Also notice that at the point of the curve the penis shaft is contracted at that point.  It acts like a broken stick. Just haven't had full erections and not able to keep it up. Beginning to be quite flustrating.  My wife is very understanding.  Any suggestions would be appreciated.  Thanks  Bill  O

Dear Bill;
Thanks for your question.
It sounds like you may have two processes going on.  First erectile dysfunction secondary to Peyronie’s disease.  I recently answered a patient with similar complaints, who was asking about correction. Please click on site search and type in “Peyronie’s”
You are not reporting pain which is variable in Peyronie’s patients.
If your angulation is not too bad you may request a medically certified vacuum erection device (VED) from your Urologist to help with your erections . The urologist may ask you to document your curvature by taking  a “Polaroid” ( self developing film) of your penis when it is erect.  This will help determine if the penis will fit into the VED. This device is a cylinder connected to a pump that has a pressure valve for protection. The penis is placed to the hilt inside the cylinder.   The  pump is pumped and a vacuum is formed around the penis and the penis engorges with blood.  To keep the blood in the penis after the vacuum is released, a comfortable constricting elastic band can be slid over the end of the cylinder to the base of the penis.  If the vacuum is not for you then penile prosthesis is the “cadillac” alternative.  There is more information and diagrams on webpages found in the patient teaching/ information section at Nephron.com.
The other problem you maybe having is called premature ejaculation. It is the most common sexual problem in men including impotence. The cure rate for premature ejaculation approaches 90-95%.  This involves a motivated couple, the “squeeze method” and sometimes a sex therapist .  You and your partner will receive instruction from a book describing the technique or from a certified sex therapist.   Instructional sexual exercises are described and then you and your mate  are expected to perform these exercises as homework. The central theme is to help you tune into and realize the impending orgasm. This helps you learn increased control. There are also some medications  which may be prescribed.
This information is provided for general medical information purposes only. Please consult  your physician for diagnostic and treatment options pertaining to your specific medical condition. Drs. Liroff, Holloway and Keoleian at our institution haves experience in and I would  recommend that you see one of them for your problem. If you would like to make an appointment, please call us at  (1-800-653-6568). We can also arrange local accommodations through this number if this is  your need. Please bring any physicians’
notes and lab test results that you may be able to obtain. These will help us greatly.
