looking for diagnosis

symptoms :
1.If I strain myself I start to get ill,It takes 6 hours then I am violently sick often up to 20 times.
2.Before and after being sick I have a fever ie. I am hot & sweaty but cold and teeth chattering
3.If I bend and strain picking object or say I am under a car at head height lifting out a gearbox I can do it but then I start to get ill & I know that appx 6 hours later I will be sick.
4.After the above I have to go to bed where I am in pain in the lower part of stomach/groin area and lower back ( kidneys )
5 I am in the garage business so everyday I was pushing cars in.
6.Strange though if I am lying flat on a weightlifing bench
doing bench presses I do not suffer any reaction Its only when I am standing or bending and straining.
7.I do have 2 small kidney stones ( which are not moving ) at present,the hospital says my illness is more than kidney stones.
8.I have a cyst on my right testicle ( for the last 20years ) which I have learnt to live with ( I think that I do have a strain here but the doctor doesnt think so.
9 I am 58 years old but fit I still do press up weights and running,My blood pressure is 100 over 70.
10. I only drink couple small bottles beer a night,I dont smoke
cigarettes but I have been smokes several small cigars a day.
11.If I dont strain as told above I dont get ill.
12.If I strain to reach a climax whilst having sex that will also make me ill.
13.The Hospital and my doctor admit they dont know whats wrong with me.

It is impossible to give a diagnosis over the internet without a proper physical exam. It also would be unlikely that I can come up with something that the hospital or physician haven't thought of already.
Pain the lower abdomen or back can be due to a variety of GI conditions - including irritable bowel disease, inflammatory bowel disease, or a colon mass. A colonoscopy as well as a CT scan can be considered for evaluation.
GU problems like prostatitis can be considered. A rectal exam to evaluate the prostate can be done, and any prostatic secretions can be sent off to the lab for analysis and culture. Infections like prostatitis can also lead to fevers and chills.
With the symptoms occuring when straining, this can be indicative of heart condtions - since straining can affect cardiovascular flow. You can consider a stress test or an echocardiogram to evaluate the heart.
These options can be discussed with your personal physician.
Followup with your personal physician is essential.
