pain after ejaculation but not every time !

Hi not sure if I have posted this in the right section but I have no idea whats wrong with me. Sometimes but not every time after I ejaculate I get a soaring pain in my penis, so bad I have to sit up or walk about and it also makes me feel like I need to urinate but when I try I cant go. The pain lasts for about 20 - 30 mins. I have noticed this happens more frequently if I have recently drunk fluids alcoholic and not before sex/masturbation. As I mentioned this does not happen every time, my local GP referred me to a specialist who tested my bladder and prostate but found nothing, and since then have not bothered to get back to me and do further tests! Damn NHS!

Can anyone please help, anyidea what I have or point me in the direction of where to go?

Thanks, Dan

Hi,

Pain and frequency during urination or ejaculation can be related to a UTI or an inflammation or infection of the prostate gland. The infective organisms may be located only within the prostate and come into the urethra during ejaculation. If they are not flushed away by urine, they would continue to be present in the urethra and try to grow there causing irritation and pain and a feeling of wanting to pass urine even if you have just done so. The irritation within the prostate would cause irritability of the detrusor muscle of the bladder causing urgency even on slight filling of bladder. Enlargement of the prostate gland can also affect the stream adversely.

You may need to have another urinalysis done, preferably from a sample collected just after ejaculation. A prostate fluid analysis can also be done. This is to identify the organism causing the infection and may be followed by a culture sensitivity test to determine which antibiotic the organism is sensitive to. A prostate ultrasound (done rectally) will give an idea of the state of the gland and whether it is enlarged.

Also have yourself tested for chlamydia and mycoplasma (STDs) as these are two infections that can cause symptoms of UTI and prostatitis but are not detected on routine urinalysis or culture.

Ensure that you keep your fluid intake over 2000 ml per day and avoid prostate irritants, such as caffeine, alcohol, citrus fruits, and spicy foods. Also avoid sitting for long periods of time or on surfaces that are not well padded. If the problem appears aggravated after alcohol intake, it could be due to the associated dehydration and can be corrected by consuming plenty of water while drinking or afterward.

If you do indulge in intercourse during the period of treatment, ensure that you practise safe sex (condoms) so as to not infect your partner.

Do keep us posted on your doubts and progress.
