I have a penile enlargement routine that involves Jelqing (milking the shaft in a semi-erect state to gradually increase girth). About 18 days ago toward the end of the session I noticed a sharp pain in the left side of my penis. Afterwards I had pain in that area on and off. My penis was also very small when flaccid and my erections were much less frequent. After a week the symptoms seemed to have abated and I masturbated to ejaculation (This was 10 days ago). I tried masturbating again a few hours later and this time again there was a sharp pain in the left side and the symptoms returned. I have tried to masturbate a few times since and as the penis becomes erect there is pain on the left side and also deeper around the ligament areas.
There is a slight 1/4 inch swelling on the left underside of the penis about 1/4 inch below the glans that is only noticeable when semi-erect. I'm not sure if this is a vericose or thrombosed vein since I have no significant blue spots or a cord development.
I have not jelqed since the beginning of this and have been using a heat pad and aspirin for the past week but the appearance and symptoms of intermittent pain and weak erections are more or less the same.
Any insight on this would be greatly appreciated.

Looking for any evidence of penile trauma or a hematoma that may have occurred during the manipulation can be considered. A simple ultrasound would be a reasonable initial test.
You can also look for infection - such as STDs as well as a UTI. A urethral swab or urine tests can be considered to evaluate this.
If the symptoms continue despite non-revealing tests, a cystoscopy to evaluate the lower GU anatomy can be considered.
These options can be discussed with your personal physician or urologist.
Followup with your personal physician is essential.
