I can understand your concern for the right sided testicular, groin and lower abdominal pain.
The first possibility which needs to be ruled out is of course of inguinal hernia. It is a result of a weakness, tear, gap or opening in the muscle wall of the lower abdomen or groin through which the contents of the abdomen, such as intestine, may protrude through the muscle creating localized pain, pressure and a feeling of heaviness.
Usually it presents as a painless bulge initially but in some cases, there is no visible bulge present, but rather mild pain in the groin area characterized by aching or burning sensations. This pain can be felt in the groin region, but may also be present in the leg, back and pubic area.
Other than that few other possibilities need to be ruled out. One of them is Epididymitis which is swelling (inflammation) of the epididymis, the tube that connects the testicle with the vas deferens. The second one is orchitis which can occur secondary to epididymitis. Epididymitis may begin with a low-grade fever, chills, and a heavy sensation in the testicle area. The area becomes more and more sensitive to pressure. It is diagnosed basically on the basis of physical examination and sometimes Doppler ultrasound is done.
The third possibility is of radiating pain due to kidney or bladder stones. But this is usually presenting with lumbar pain and urinary symptoms as well. In young males, often excessive bicycle riding and prostatic infection can cause such kind of testicular pain. Sometimes sitting on an inadequately padded surface or for prolonged periods of time is causing transient irritation to the prostate which is already inflamed. This could be appearing as the pain in testicles.
Hence it is mandatory to get a few investigations done. These include ultrasound lower abdomen, testicular ultrasound and color Doppler, urethral swab and PSA (prostatic specific antigen). Best would be to get these investigations done from a urologist. An ultrasound of the testicles and lower abdomen will give a more clear cut picture.
