It is possible but unlikely that hydroceles would be the cause of painful ejaculations. I would first rule out infection - which can be done with a urethral swab or urine test. I would also do a thorough prostate check to evaluate for any infection.
Anatomical abnormalities - such as urethral strictures or polyps - can also cause these symptoms. A cystoscopy can be done to evaluate for these possibilities.
I would suggest a referral to a urologist if not already done. I would discuss these options with your personal physician.
