penis pain

Hi, I have mentioned before that I have a fistula between my prostate and rectum and that I have a suprapubic cathter. Although it has been uncomfortable at times, I now eperience pain in the penis and area of the scrotum and rectum.
With my last consultation with the urologist, I was told that the fistula was small and that I am to wait till March 2010 to see what will happen over that time. Can the fistula be left as it is or does the hole have to be closed?
Also I believe that the pain that I experience is due to either the suprapubic catheter or the result of the fistula.
Should I contact the urologist earlier then March 2010?
