penis retraction and Incontinence

  My doctor and I are having difficulty finding out a title to put on this and how to treat this five year long desease. I am 42 and I sometimes wet the bed. My penis has retracted to what appears to hide inside the skin behind the testicles. I am also impotent. someone ask me questions help me answer this. I need to get back to normal, five years is a long time.
