possible kidney infection? what else?

Hi doctor,

On May 17, I had some blood work done. I had felt like **** for a month and a half - some genital area discomfort, muscle and joint aches, mild fever...and so on. The doctor checked me out and told me I was healthy....I was worried about STDS.

The test came back positive for:

1. Mono
2. Increased kidney function (I was urinating frequently)
3. Decreased lymphocytes
4. Two separate doctors also diagnosed me with prostatitis.

I did not respond in any noticeable way to Cipro.

I had been concerned about STDS from an unprotected sexual experience with a girl in early January. (I am male.)

on May 1st (3 and 2/3rds months later) I tested negative for HIV, Chlamydia, Gonorrhea, and Syphilis.

However, it is now August and I still have some joint pain. Genital discomfort. For example, sometimes I know I have to urinate because I'll get a pain in my penis. I've had all sorts of symptoms - pains where my liver and spleen would be, kidney pain, leg pain, weird pains at the back of my head, dry skin, some rash (which is less frequent), foot pain, etc...

I have had every prodromal symptom of herpes and most ARS symptoms. Curiously though, I have never had a sore throat, lymph nodes were never swollen.

Also...a few weeks ago, I drank two beers. It felt like my kidneys had shut down. I drank a lot of water after and didn't have to urinate for six hours despite the beer.

I believe I am having trouble fighting off mono because my lymphocyte count is down.

My doctor has not suggested any further testing and doesn't know what could be wrong.

1) what could be wrong with my kidneys?

2) should i retest for hiv or any other std? (i am trying to take the test at 110 days or so as 100% accurate)

3) what could i possibly have that would cause my lymphocyte count to be low (17.6)? (White cell count = 6.7)

4) should i test for anything else?

There is no further exposure to STDs. Does this mean you do not believe further STD testing is necessary?

Though I should mention that I have something resembling thrush on my tongue and occasionally something that looks like geographic tongue on my penis; sometimes a red patch -- which clear quickly.

To answer your questions:
1) There are a variety of diseases that can decrease kidney function.  Obtaining a urine sediment test, imaging the kidneys and repeating the blood creatinine level should be considered.

2) Repeating the HIV and STD testing can be considered if there is still exposure.

3) Infection can lead to a low lymphocyte count.  If the count continues to be abnormal, obtaining a blood smear or bone marrow biopsy can be considered.

4) I would image the prostate to look for an abscess.  This can cause continued symptoms that would not be treated with antibiotics.
