pyeloplasty follow up

My 9 year old daughter was diagnosed with bilateral hydronephrosis
while in utero.  She had pyeloplasty done when she was 7 days old.
She was followed closely at first and then every 2 years.  Every
visit she had a fancy test done.  The last function test done 4 years
ago revealed 40% function in one kidney and 60% in the other.  Since her last
visit, her physician had died. We just had our visit with the doctor that
inherited her case and a renal ultrasound.  The doctor came in and said her
kidneys remained the same and no more follow up was necessary.  I was really
taken aback that no more visits were needed. Is this standard protocol?  We
were told that when she hit the puberty stage that the kidneys may have problems with the strain put upon them.  I feel we have been deserted if we
are just around the corner from possible problems. What is your opinion on
follow ups and any problems with puberty?

Dear Wendy,
The history you have outlined is very general in nature and , therefore, it is hard to make specific comments, however, it seems that your daughter has done well during her long follow-up of 9 yrs.  In this context, it is possible that no major changes have occured to warrant any further concern.  It is , though, not unreasonable to follow the patient with an ultrasound every 2 to 3 yrs for a few times before stopping the follow-up altogether.  You must continue to follow with your primary doctor with your daughter
