Difficult to say without evaluation. The groin area is quite moist and is an ideal area for a fungal infection.
If that is the case, it would be unlikely that cortisone cream will help.
I would have it seen by a physician. If the diagnosis is indeed a fungal infection, various antifungal creams or powders can be tried for treatment.
