Hi, i'm 49 years old and i have this problem simce Last aug.2004 i developed jock itch in groin area. Over counter demerex dm cured that...in the meantime i was feeling a ennoying, slight burning at tip of penis 12-24 hours after ejaculation...no problem urinating, no bumps, rashes on penis. After 24 hours -36 hours i would feel a very itchy feeling inside the urethra from the bottom to the top of my penis and some anal itching when this flair-up happens..also when i did ejaulate about an inch inside before the opening it would feel very sensitive...as the fluid was coming out....these symptons last 5 days and then go away..until the next time i ejaculate

I visited my doctor..he check my blood, urine analysis, said it was urethritits put me on doxycycline for 2 weeks. That made no change...nizoral for 10 days, no change...I went to a urologist...checked my psa..  0.50, physical examination of my prostate normal..urine tests, culture and yeast urine test..normal..the urologist said to me you do not have a prostate problem but i am going to give you this drug called proscar anyway..this will cure any problem that you do have..Well it didn't....same problem after ejaulation only....

This problem is hurting my relationship with my girlfriend..I have been retraining from sex because i can't go thru 5 days of agony...This only happens 12 hours after ejaculation..otherwise i'm fine..no problems urinating, no pain....I did notice 5 days ago when i masturbated and ejaculated the sensitive area 1" below the head was not there...which i was happy about..but this time it took 2 days for my symptons to start...could this be some type of fungal bacteria in my prostate?? It all started with jock itch..or some type of tenia ..I never had this before or jock itch before..I do swim in a river near my home..Boating and water skiing..Could i have got some kind of parisite???

This is driving me crazy.....i've been thru 4 doctors..My regular internist, 2 urologists, and one endocrinologist.
They all said they never heard of a problem like this...

What is my next step...?????

Although probably already done, I would test for STDs such as gonorrhea or chlamydia.

Assuming they are negative, the prostate would require more thorough evaluation.  Prostatitis can certainly manifest as these symptoms and can be difficult to detect or treat.  You may want to consider a transrectal ultrasound, which can image the prostate for any abscesses.  If present, it would have to be drained since antibiotics would not be effective.

If negative, you can consider a cystoscopy - since anatomical abnormalities such as a urethral polyp or stricture can lead to the symptoms.

You may want to discuss this option with your personal physician.

Followup with your personal physician is essential.
