I would obtain a scrotal ultrasound to evaluate the anatomy and structure of the testicle. Conditions like torsion may be present (although the presentation is more acute), but deserves to be ruled out given the history of trauma to the area.
Although less likely, I would also obtain a urine test to look for infection - which can also present with testicular discomfort.
It is possible that the scrotal pain can be referred to the rib cage - although I am not familiar with this particular symptom. If it continues, you may want to consider a neurology referral.
