Difficult to say without evaluation. I agree with the ultrasound. You may want to confer with a urologist that the hydrocele isn't causing the pain. More detailed imaging can be done with an MRI.
If the pain continues, you can consider a cystoscopy to look for a urethral polyp or stricture that may lead to the discomfort.
