  My son has mixed gonadal dysgenesis 45,x-46,x dic (y). One area of concern mentioned was the increased risk of testicular cancer. His genetalia are of a normal male although he is lagging behind in maturation. At age 15.75, he is at Tanner stage III with bilateral testicular volume of approx. 10-12 cc. Testosterone levels <0.35 nmol/L. Here's the question. Just how much of a risk is there for testicular cancer? We have had such a wide range of recommendations from biopsy to examinations q 3 mos. We do not want to get overly alarmed in this issue yet want to be knowledgable. We are aware of the use of gonadectomy in Turners Syndrome girls, but I don't think this applies in our son's case. What particular variables make our son more at risk than the reat of the population, and what would you recomend as reasonable preventative steps. Thank you.

Thanks for your tough question.
The problem with deciding what is best for your son depends on two issues.  First, what is the risk of getting a tumor.  Gonadoblastomas ( type of testes cancer)  are rare tumors in the general population  but are associated with gonadal dysgensis and more specifically the  Y chromatin.  Female outward appearance and intra abdominal testes are also risk factors.   Your son
