testicular cyst and sperm granuloma

I'm having surgery next month to remove a sperm granuloma, because i have had severe pain on the right testicle since my vasectomy for year's now.  Recently a ultrasound revealed a 4mm cyst on the left side.  I only have a little pain on the left side , just when the right side with the sperm granuloma is killing me.  My doctor think's the pain on the left side is because of all the inflamation (inflammation) from the sperm granuloma, on the right side, not the cyst.  So he think's the cyst is not the source of the pain since it's so small and it's best to leave it alone.  I'm just wondering if i should get a second opinion.  I would hate to take time off work to remove the sperm granuloma, and then six month's from now still have pain and have to remove the cyst.
