Finding a Doctor Post Mesh Surgery

Hi, I had the transvaginal mesh surgery in 2009. I have since started having discomfort. I cannot find the original physician who did hte surgery and have also moved from Washington State to Idaho. I am curious the best way to find a new doctor and or locate my old one so I cna have things checked out and repaired. Thank You.
