I am no added salt, control sugar and starch, I can't seem to loose  weight.  How do I accomplished a 40 pound weight lose

Sorry about the pacemaker.  Barb may be right about there being too many carbohydrates in your diet.  But you may want to check with a clinical nutritionist to see how much protein is safe for you with the pacemaker and your hypertension.  Also you don't say how strenuously or how much you are able to exercise.  Also I'm sure that the Coumadin and the hypertension meds may cause you to retain water.

What makes you think you are a diabetic?  Have you been having symptoms like frequent urination, extreme hunger and thirst, constant fatigue, blurry vision, etc?  I am a Type II diabetic, and when I was diagnosed the symptoms were impossible to mistake or ignore.  All your doctor has to do is take your fasting blood sugar. If you test over 126 mg/dL 3 separate times, you are a diabetic.  If you test between 100-126, you are insulin resistant, which means you can still do something about it.  Usually a A1c test is given to determine how elevated your blood sugar levels have been over the past few months.  Sometimes they do an Oral Glucose Tolerance Test instead.

Even so, when I was first diagnosed with diabetes, the disease, my meds, and having to adopt a low glycemic lifestyle caused me to lose 50 lbs very rapidly.

There are plenty of vegetables, other than the green leafy ones.  You can eat green beans, broccoli, cauliflower, squash of all kinds, tomatoes, cucumbers, peas, corn (watch the peas and corn because of starch).

It sounds like you might not be getting nearly enough protein.  While peanut butter is an excellent food to provide protein, 1 tsp is not nearly enough.  You could try a couple of eggs, with your toast or a glass of milk - those are both high in protein and vitamin B12.  

You should make sure you  have protein with every meal/snack. You can get that in the form of meats, dairy (cheese, Greek yogurt, milk, etc), eggs, nuts, beans.  

When you fix your plate, it should be 1/2 full of vegetables, 1/4 of protein and 1/4 whole grain, with dairy on the side.

You also have to make sure you take in enough calories or your body will think there's not enough food and will hold onto fat stores.  

There are a lot of herbal teas that have no caffeine.

Thank you for your responses.  I have a pass maker, on coumadin, blood pressure medication and I could be a  diabetic but I watch what I eat and exercise.  With the taking of coumadin you have to watch the green leafy vegetable.  I shop at Trader Joes and I find the no salt added food there.  I find it very hard to lose weight.  I am not on a diuretic medication so I have to watch the salt diligently.  For breakfast I would have two slices of No Salt whole wheat bread.  A teaspoon of no salt peanut butter and a cup of Hot Water (can not drink anything with caffein.  Please give me ideas to help me loose this weight

You could have your thyroid levels (TSH, T4, T3) checked, but it probably isn't connected to this.  You mention no added salt, control sugar and starch, but you don't mention how you do this.  What you are eating and how much definitely matters, along with how much water and exercise you get.  My advice to another member was : You can possibly come close to 10 lbs. a month if you stick to your plan and exercise regularly.  My advice would be to opt for lean protein and non-starchy vegetables and cut your carbohydrate intake.  The carbs you do eat should be whole grain and try to eat them early in the day so you have time to burn them off.  Yes, drink lots of water to flush your system and add some flax seed oil to boost your metabolism.  You do have to be careful of fruit intake and stay away from sugary ones like bananas. Remember, low glycemic.  I lost 50 lbs. this way about 7 years ago.

P.S.  I also wore a pedometer and tried to get around 5,000 steps a day.

Do you have medical conditions that make weight loss difficult?  

Those might include insulin resistance, hypothyroidism and/or PCOS.

If you do, you have to get those under control before you will be able to lose weight.
