I am extremely overweight. Need to lose a lot fast to get started

I am really overweight.  I need to loose at least 200 lbs.  The problem is I am almost completely home bound.  I can't hardly move due to severe back pain.  My muscles in my arms hurt.  My feet are numb and tingle all the time.  The muscles in my legs burn when I do walk.  The Dr thinks that I might have a herniated disc and thinks that I should see a neurologists to rule out MS.  I also have eye problems, dizziness, I had painful needles and pins in my hands that have resolve plus feeling of rubber bands around my wrists but because I don't have insurance nothing has been done.  

So I need to loose a lot of weight fast.  In my mind if I drop at least 50lbs I should be able to get around enough to do something.  Then I could finish rest of my weight lose reasonably.  I know this isn't realistic but right now even the slightest movement hurts.

Do you have any sugestions on how someone can loose without much exercise.  I have tried phen.  through my Dr but just couldn't take it-it actually made me sleepy and irriatiable.

  i took phentermine prescribed through my doctor. at first started noticing my eye sight was different i just thought i needed to get my eyes examined and get new glasses. i went to my doctor to get weighed and bp taken. my bp was very high. i went to get my eyes checked and when my right eye is covered i cant see out of my left eye. i can see brighness or movement but would not be able to read anything. i have been diagnosed with a eye disease which very bad in my left eye. i think the med progressed my eye disease to were its very bad in my left eye. what is suppost to help with the eye disease didnt work so i have had to go see a neurologist to see what else this med has done to me neurologically. if going to take this med do it through your doctor or a reputable doctor. when my doctor prescribed the med and got it through a pharmacy didnt cost me an arm and leg to get the med. if i had know this would happen to me i never had taken the med. whatever you do becareful with this med. if you having medical problems already i dont know if phentermine is for you. do you have a doctor that can go to your house?
