My nipples are very painful
My nipples have been sore for about a year.  They are very sore painful and painful whether I am on my period or not. I have always had very sensitive nipples, but now this they are painful in a bra, painful when even a sheet touches them. I am not pregnant.  I have regular periods. I am not on birth control, and I'm 29 years old.  
I have not done anything differently, I sleep on my stomach and sides. When I am lying down my breast do not hurt until I move or tough them with the sheet or the gown I have on. The water in the shower hurts and to touch them hurts, there is no discharge or blood when I squeeze them. It Hurts!!!!!! I have had mammograms and ultrasounds and nothing has been found.  I cry when my husband touches them.  I do have big breasts, 36 H. I've even thought of applying lidocaine on them just to ease the pain.

I would go a get your prolactin levels checked. It may be that those levels are high and your body thinks it is on the verge of breast feeding - it can be very very painful.
It can happen from pituitary tumors, thyroid imbalance and other reasons like medications - so get it checked out.
