Pain during period

I'm 26, over the last few years i've been getting really bad stabbing pains inside my vagina while i'm on my period, is this normal?
The pain is worse just after I urinate and change a tampon, I change them frequently, especially when it's heavier, so I dont think this is the problem. Sometimes it's so bad i end up bent over for 5mins just to ease the pain, it wears off a little bit but is always there during my period. These are not normal cramps, these pains are right up inside my vagina and it fells like someone is stabbin a knife inside me.
Also the right lower side of my abdomen sometimes swells up and feels like a lump, I went to hospital about this and they couldn't find anything. Also where this 'lump' is while i'm on my period it feels like my skin is being stretched, sometimes ripped when its worse....are these just normal girly pains or should I go back to my doctors?
