Perineal Hernia

by Wiggles, Mar 28, 2006 12:00AM
I have two large bulges in my along side my vulva area. It started 15 years ago after I delivered by son, but through the years and after menapause it has worsened. My MRI results showed that there was a prolapse of the bladder into the right vulva through a defect in the right levator. The findings of a urogyno
doctor stated that the vulva has markedly distorted architecture
with a protuberance of the labia majora that descends well below the ischial tuberosities.  It has an hourglass contour with obvious peristalsis noted. This is within the labia majora but also moves psteriorly involving the ischiorectal fossa.  On palpation, and with an attempt to interdigitate the hernia sac, it is not clear that it involves the right inguinal canal, and there does not appear to be a right inguinal or femoral hernia. The pubococcygeus is 12/12 on the left but the pubococcygeus and puborectails are absent on the right although the iliococcygeus is intact. The external anal sphincter is intact and concentric with a strenght of 12/12. The finding was that I have a fairly large neck hernia into the labia majora  and ischiorectal fossa on the right. This appears to be an intraperitoneal process and does not appear to communicate with the right inguinal canal. It is difficult to see whether or not the hernia sac is filled with bladder or small bowel on the MRI.
Dr. recommended a hysterectomy with a fairly extensive pelvic dissection, a sacral colpopexy, perivaginal repair and possibly retropuic urethropexy or medical management. Are there any exercises that would help me to avoid surgery?

I don't really think exercises like Kegel will help with all of that. I just had a vaginal hysterecomy with colpopexy, cystocel and rectocel repair....now after you are healed then I would think the dr would like you to do Kegel exercises...but you would need to make sure they are safe for your particular situation.  I am almost 6 weks post op from all of this....its not easy and yo must have help...........but its better than the prolapses I had.
