What can cause intense itching in both the anal and vaginal regions?

Help! My nether regions won't stop itching! I've been to the doctor and they've said it was just pruritus which a common butt issue apparently. But now the itching has spread to my vaginal area. Worse, I've begun developing painful abscesses in both areas, one of which I've had lanced. I plan to return to the doctor, but I'm worried that they'll just prescribe anti itch meds or antibiotics, both of which I've already tried and neither of which worked. What do I do?
