adhesions and endometriosis

Doctor,
I am 40 yrs old and I hv one daughter aged 19 yrs... i had two abortions at the age of 23...I had  multiload inserted for 9 yrs (23 - 32) replaced thrice for every three years till 2000... i am not following any precautionary methods from 32 onwards...and from 32 onwards i started getting heavy lower abdominal pain during my periods... i was taking painkillers as per my gynaecologists advice..after five years of struggle during periods, painkillers dint help me to reduce pain so i use to take injections during my periods with my doctor to reduce my pain..but my periods was quite regular and very normal bleeding and last for 2-3 days... my doc told me that it could be endometriosis problem and started contraseptive pills to control the hormone system.for  3 months and stopped.. i was better for some months after that... but in aug'06 - first time in my life i had sudden pain only on my left lower abdomon and back which increased to the high and it was unbearable but i was not having periods during that time and went to hospital for immediate ailment, it was found in ultrasound that i had 8.7 cm cyst on my left overy but it was haemarraged with internal echos... so they started contraseptive pills which helped me to dissolve the cyst within a month.... from that time onwards i started getting pain even without periods and it was found in ultrasound every month, that there are cysts on both the overies sometime haemorragic with internal echos... i was taken for laparoscopic surgery in jan'07 and found that uterus is not visible, and it was fully covered by bowel and severe PID with adhesions... they could not proceed further so they just did tranvaginal aspiration of the ovarian cysts.. after that i started contraseptive pills to control my pain for 60 days but i was frequently getting severe pain and  often clots of blood discharge even before and after my periods... sometime the bleeding is high and i use to take tab.tropic to stop my bleeding...my recent scan reveals left hydrosalpinx, since the pain is high and bleeding is irregular i was advised by doctor to undergo total hysterectomy with the help of gastroentrologist to remove adhesions...my gastroentrologist said that he will just make the way by pushing the bowel on uterus to access uterus. pls help me to clear my queries - is this surgery risk or is there any other treatment to avoid pain without surgery... some doctors tell that severe endometriosis along with adhesions is risk to do hysterectomy....will there be any bowel related problem after surgery... pls guide me.... am really confused and scared of my problem...

By the way, there is no doctor on this particular forum.  Try the gyn forum on MedHelp for a doctor's opinion.  Good luck!

I don't think anyone can guarantee that there will be no bowel related problem after the surgery.  But there is a bowel related problem now, and your symptoms are so severe that I cannot help but think a hysterectomy or at least surgical removal of the ovaries so your hormones don't cause so much pain would be helpful.  If you need more guidance, you really need to be consulting a doctor, a different one if you don't trust the ones you have spoken to.  But I have always heard that endometriosis is so terrifically painful that unless you think they can do a surgery to remove all the scar tissue and all the endometriosis, you are going to be stuck with a painkiller regimen that is probably not good for you and doesn't sound like it is working, unless you do something to reduce the hormone load.
