Hello,
If you are having pain while inserting the tampon then infection of the vagina causing vaginitis or cervicitis should be ruled out. Cervicitis is most often caused by an infection, usually caught during sexual activity. However it can be due to use of diaphragms, allergic reaction, pessaries etc.

You should get a pelvic examination done from a gynecologist.
It is very difficult to precisely confirm a diagnosis without examination and investigations and the answer is based on the medical information provided. For exact diagnosis, you are requested to consult your doctor. I sincerely hope that helps. Take care and please do keep me posted on how you are doing.
