There has to be a free clinic near year, isn't it the law in the US?  Are you eligible for state assistance since you are raising the grandkids.  You  have to take charge of your health.  Are you at a healthy weight?  Are you getting exercise?  Are you following the proper diet to manage your diabetes?

My Mum was 40 lbs overweight and on all sorts of meds.  She lost the weight and is now off all drugs.  She saw a nutritionist and walks 30 minutes per day.

If you blood pressure is that high you can just go to the ER - they have to help you if you come. Also, I would good to public health.. Maybe your state has something to offer.

Luckysue,
I don't know what type of diabetes you have but try to elimanate carbs from your diet as much as possible it will help with it.  Also, if you can't work you need to apply for state assistance--then you will have medicaid and be able to be seen by the doctor and get your meds--I wish you luck.

yeah, anxiousmomtobe? is right. Nutrition can solve tons of problems. IF you can't get on meds for your problems I would try checking out a book at the library about managing your illnesses. Try to make walking a part of your routine.
