Idab - I came across another thought. Have you been evaluated for Guillain-Barre syndrome? It fits all of your problems, initial gastrointestinal infection, subsequent weakness/paralysis of arms and legs (usually legs first then arms) and it affect the upper legs and upper arms before the lower. Many have severe paresthesias and PAIN. Also difficulties swallowing, speaking are not uncommon. May improve, relapse. There are variants, but you should read this article. It's written for doctors, but you've been through so much you'll likely understand it.
