Mildly Elevated Serum Calcium with a normal PTH

Hey all,

Since September 2006, I have been having problems.  Initially it started out as leg and arm weakness accompanied by muscle twitching.  An EMG and NCV ruled out all bad things and I was diagnosed with benign fasciculation syndrome.  When my GP ran blood work in Sept. ’06, my calcium was slightly elevated at 10.7 (Ref. 8.8 – 10.4).  My hemoglobin and hematocrit were also elevated.  I am seeing a hematologist and receiving phlebotomies to get my red cell count down.  Over the past 9 months, my calcium has been tested several times and has ranged from 9.8 to 11.  Every time it was tested, my PTH was normal (usually in the 40’s) (Ref. 12 – 65).  I have had renal ultrasounds, pulmonary function tests, SPEP and UPEP (tests for multiple myeloma), MRI of brain and cervical spine, lumbar spine x-ray, chest x-ray, ionized calcium, 24-hour urine calcium, and tests for Paget’s Disease.  All tests were normal.  

Currently, I am experiencing pain in my mid-back area, and pain in my ribs.  I am always tired and am having trouble sleeping.  I wake up about every two hours.  I am having my calcium checked again this week, and if it is high again, I am going to an endocrinologist.  My GP believes he has ruled out most of the bad things.  I am leaning towards hyperparathyroidism, but my PTH is always normal.  I am glad it is not low though, because that usually points towards cancer.  Does anyone have any idea as to what is causing my calcium to fluctuate from high normal to mildly elevated?  Also, why are my ribs and back hurting?  I am really worried about myeloma, even though that has been ruled out by tests.  Plus, after 9 months, with no treatment, I am sure I would be worse off.  Any input is greatly appreciated.

Just to eliminate something really simple and silly, are you taking vitamin supplements?  An overabundance of vitamins A or D can cause hypocalcemia without affecting the PTH.

I am not taking any supplements.  Believe me...the doctors tested for everything.

Tarrigo,

What was the outcome of your case?  My two blood tests revealed normal PTH scores (22 and 35) but high calcium (11.1 and 10.8).  Multiple Myeloma, humeral cancer, and familial hyperparathyroidism has been ruled out.  My endo is confused and I need help!  

Anyone have any suggestions/comments?

Please check out the site www.parathyroid.com.  The site should answer most of your questions.  High calcium is a parathyroid malfunction even if the pth level is normal.

Oh, believe me, I have read every word on parathyroid.com!  Just curious to know how Tarrigo turned out, as his/her post was in the summer.  Are you out there Tarrigo??
