Hi Doc,



I am 32 y/o married man. I tested HIV- before I married 3 years ago and have been monogomous up until this point. Unfortunately, I did something out of my character 8 weeks ago.



I met a supposedly HIV negative man online. His profile indicates he is HIV- and I also asked him before doing any sexual act that he was HIV-. I have never been with a man sexually before then. I WAS bi-curious before this encounter .. the anxiety that this encounter has brought shows it is not worth it.



He performed unprotected oral sex on me. He never inserted his finger but rubbed my anus during oral sex.



He then wanted to have me perform anal sex on him. He inserted my penis without a condom for a good 20 seconds before I realized what I was doing. I pulled out and asked for a condom and proceeded with anal sex with a condom for about 3 minutes before I decided I did not like it. I left his house at that point. Am I overly concerned about STDs?



I do not think I have any symptoms of ARS. However, I did have penile discharge after 5 weeks and went to an urgent care clinic. I was tested for chlamida and gonorrhea and given 1000 mg zithromax and 500 mg of cipro. However, the test came back negative to either STD. The dr tested for trich and said there was white blood cells in my discharge but I was negative on the trich test. The discharge stopped after 3 days and my symptoms seem to clear.



After the urge to frequently urinate came back, I went to see my PCP last Thursday. He just did a urinalysis and said I seem to have a bladder/UTI infection (based upon white blood cells in my urine) and prescribed antibiotics. I told him about my tests at the urgent care clinic so he did not repeat the STD tests that I took 3 weeks prior.



I have read this forum over and over but wanted to get confirmation from you about my risks and where to go from here.



1 - Hopefully, that guy was HIV-. However, if he was HIV+, what are the possibilities of me getting HIV? He never ejaculated during the entire sexual encounter. Nor did I touch his precum.

2 - I did not have any STDs prior to eight weeks ago. However, my penis was inserted during anal sex without a condom for a brief period. Am I susceptible to HIV because of the lack of condom?

3 - You have mentioned that the risk is very low for oral sex transmission and rubbing around my anus. Correct?

4 - What other STDs should I be tested for?



My doctor indicated HIV is not prevelant in our area except for needle drug users or very, very promiscous gay men. I have a feeling this man is very promiscous hence my suspicion.



Any advice on calming my fears. Neither the urgent care Dr nor my PCP said a HIV test will confirm any findings until after 6 months. Should I go to requestatest.com and ask for an HIV test? Or wait until 6 months and go back to my PCP for a HIV test?



I am worried that I will pass HIV onto my wife and/or any other STD. She is going to her OB/GYN next week.

Your risk was low.  HIV is transmitted rarely by oral sex, especially to the insertive partner from the oral one; and certainly cannot be transmitted by hand contact with the anus or any other part of the body.  That you acquired nongonococcal urethritis (NGU) afterward is of some concern, but I am not at all certain you acquired it from that sexual encounter; NGU generally shows up within 2 weeks, not 5 weeks later.  On the other hand, normal oral bacteria can cause NGU, acquired during oral sex, with negative tests for gonorrhea and chlamydia.



it is doubtful you have a non-sexually transmitted UTi/bladder infection, which is rare in men under age 50.  Most likely it is residual inflammation related to your previous NGU.



1) Most people who say they are HIV negative really are.  But even if he was positive, you were very unlikely to become infected.  Look at it this way.  Among gay men who have unprotected anal sex with up to 100 or more partner per year, only a few percent get infected each year.  With those numbers, how risky can any single exposure be?  Of course people can be unlucky and get infected with only one encounter.  But you have to be REALLY unlucky.



2) Of course you were at higher risk than you would have been with a condom.  But your risk still was low.



3) Correct.



4) None; you already have been treated.



Regarding timing of HIV testing, your PCP and the urgent care doc don't know what  they're talking about.  Any test after 4-6 weeks is 100% accurate.  See numerous other threads on this forum; search for "time to positive HIV test".

I'm not jumping on this thread.  I just got done reading many of your posting, and I simply wanted to say that in my humble opinion:  you truly are awesome!



I hope you know that folks like myself do not take your services for granted.  I know you answer the same questions over and over again.   And for that, I apologize.

Hi Doctor -



Thanks for your words of encouragement and the knowledge that you deliver.



Thankfully - I had the PCR DNA test done this week (almost 9 weeks after exposure) and found that it was negative. I will retest at 13 weeks with the normal ELISA test since I already paid for it.



I know you don't encourage such a test but it brought piece of mind so I can get on with my life. I just hope the regular test comes back negative as well.



Again, thank you - I hope you know what a service you are providing even though you get asked the same question over and over.
