HIV am I a gonner

On a Trip to China met female(non-hooker)had sex(unprotected) found out she was on vacation and lived in a part of china known for HIV outbreak(worst in all of china).  Not good.  Mostly it is among IDU users (about 84 percent of all IDU users have HIV there)The IDU people live near the opium routes(she lives near these routes also, western china) but HIV is starting to bridge to normal population.  Also the strand that is out there is some b/c varient I think.  10 days past since my non-condom sex with her and now I have a pain in the tip of my penis usually hurts after  urination and before, I would call it irratating.  Accompanied with that is a nice little milky fluid a little more transparent than seamen that is present at the tip of my penis sometimes.  I'm convinced I have Chlamydia or Gonorhea or something.  But I am really worried of course about the hiv.

Questions:  Will the normal test for HIV suffice for me considering that the HIV could be some weird new varient.

Immediatly after ejaculating I quickly pulled out and cleaned up in the shower does this reduced my risk at all.  In other words is it easier to catch Chlamydia/gonorhea than HIV.

What tests should I take and when.  when is the soonest I can take a hiv test(even if it isn't 100% accurate) for peace of mind.

Please put a word in to the man upstairs I am really worried.

Thank You

PS i am going to tatoo my penis reminding me to wrap it up

The risk of HIV infection from a single exposure through vaginal intercourse averages, as you note below, around 1 in 1000.  In response to a question in the follow-up dialog, HIV is much less transmissible than gonorrhea or chlamydia.  Most persons seem to think that simple exposure to an infectious agent is all that is required for transmission to take place.  Not so.  Some infections require much more intense or repeated exposure than others.

Your penile symptoms sound like you have urethritis, such as chlamydia, NGU, or gonorrhea, but your follow-up post indicates you have now been treated for those possibilities.

The standard HIV blood test usually is positive by 4 weeks, and that would be a reasonable interval for a first test; or even 3 weeks, if you like.  Assuming a negative result, which is what I predict, you could be tested once again at 6 weeks for a definitive result.  In the meantime, don't worry too much; the odds are very strongly in your favor.

Good luck--  HHH, MD

Follow up went to Dr. today he tested for chlamydia and Gonorrea with a q tip swab then gave me a shot for the Gon. and pills for possible chlamdyia.

He went onto to tell me HIV was a very big risk and very possible i got it and need to get checked out.  I'm obviously going to get it checked out seeing as after he looked at the discharge I had coming out he said I had something for sure.  Is it as risky as he is scaring me to believe I know there is a chance but I am in nervous shock should I be.

I'm going to get a test tomorrow for the HIV and other stuff how accurate is the HIV test 14 days out(percentage)?

The Doc here has said here (and the CDC also says) the risk assigned to a male getting HIV from a woman by unprotected vaginal sex is 1/1000.  And this assumes the woman is HIV+.  Since you don't know if she was HIV+, I would think your odds would be lower.  Plus this was a one time exposure which makes it even less likely.  The odds are very very much in your favor.  If I were in your shoes I think I would test at 2 weeks, 4 weeks and 6 weeks.  Assuming all of those are negative I would take my negative results and move on.

Again, the odds are really in yuor favor.

Are you still in China?

No I came back from Beijing a week ago.  It was an awesome place-just don't go to the bars because if your non-Chinese the women will through themselves at you and I couldn't resist.

Thanks for the info!

14 days is too early for an antibody test.  Its possible an elisa might be positive that early, but it will probably be negative which will offer you no reassurance since its too early.

The 1/1000 statistic is just an average, your risk could have been much less or much more than that depending on many factors that can't be known.  The odds are overwhelmingly in your favor so try not to freak out until you can get a test at 6 weeks.

Don't beat yourself up about the incident.  I doubt I could have turned down easy sex with a hot to trot chinese girl either.  We are humans and fallible.  Next time, if you can't turn the sex down, just use a condom so you wont have this anxiety afterwards.  Its easy for me to say, because I've done the same thing.  Sure it feels much better without a condom but after you climax, you're left with nothing but fear.  I think some people actually subconciously enjoy the thrill of taking the risk of unprotected sex.  It makes it even more thrilling, but then you have to ask why would I do something like that to myself?  Let this fear stay fresh in your mind next time you're tempted to do something this risky.
