Covering bases on Trich / Chlamydia

Thank you for such a great resource.

To the point.  I had unprotected vaginal sex with a female acquaintance about 10 weeks ago.  Since then I have experienced at times some very, very slight burning of the urethra.  About four weeks ago, I had a urine test for Chylamdia/Gonnahrea and it came back negative.  Three weeks ago, my Dr. prescribed seven days of doxycycline (which I faithfully completed); however I sometimes still have a very slight burn during the first portion of my urine stream.  No discharge has been ever been noticed.  I had Chlamydia 20 yrs ago and this is not even close to the burn during that event.

My concern is I have been dating someone for eight weeks and want to make absolutely sure I don't pass something on to her.  We have not had intercourse, but did engage in oral sex once recently after the doxycycline treatment was complete.

1). Would it be wise to be treated for Trichomonas just in case?  2). Can Trichomonas be transmitted orally?  Most of the sites on the Net don't mention it as a form of transmission, but a couple does mention it.  I suspect they are simply covering everything as a Precaution.

Your insight is great appreciated.

It is possible that 'slight burning' could be due to chlamydia, but most men with such a symptom typically have no infection of any kind; it's perhaps the most common symptom of genitally focused anxiety, although that of course is a very imprecise diagnosis.  In any case, you can rely on the negative tests; and in the exceedingly unlikely chance the test was falsely negative, doxycycline would have cleared it up.

Trichomonas probably is equally unlikely.  It is only transmitted by vaginal sex, and maybe anal sex; to my knowledge oral transmission has never been suspected (although it also has never been studied).  Rather than being treated for trich, you might just call your 'acquaintance' and ask if she has any vaginal discharge or any known infection.  If not, the chance is even lower.  On the other hand, treatment is simple (a single dose of metronidazole) and wouldn't hurt.  But if you do that, you should not expect it to make any difference in yorur continuing mild urinary discomfort.

Good luck--  HHH, MD
