PAT and PVC's

Just had PAT attack and taken to ER.  After a few hours heart went back to NSR.  I'm 48, had a St. Jude MV replacement 4 yrs. ago.  After surgery was in AFib and tried meds but didn't help.  Cardioversion worked.  Last Sept. went into A-flutter, meds in the hosp. helped to convert in 2 days.  2 mths later to the date Nov. 03 -went into A-flutter again, this time they performed an ablation which seemed to work. April 5th, had a PAT attack and ended up in ER. Trying to get another appt. w/ my Card. After the ablation, electrophysiologist said he didn't want to ablate the PVC area since it's one foci area, could cause other areas that are dormant to come alive. Have 1 back to back every 30 seconds or so. Sometimes they can hit 1 after another, really burns me out quick and gets the nerves to act up. Is there anything that can help PVC's to stop hitting so hard and often? Have read posts about diets etc.  I don't drink, smoke, or do drugs. Use to be active, 6'0 160lbs.  Normal HR is 72-76. Have tried atenolol but made me very sluggish, caused more problems than helping. Had MVP since 16 and needed new MV at 44. Would this long period have damaged my atrium enough to cause more chances for Tachycardias to take place? Do you think ablation for my PVC would cause others to start? Have had holter monitors, nuclear stress test, etc. and doc doesn't think anything that bad.  It's funny because when these things happen, something has to be creating them. Last question - If I was your patient, what advice would you give me, besides see your doctor?  Thanks, RW21

what a fantastic site. triple by pass 1977 at age 47. mild infarct. 1989. angioplasty 1993. paf since 1977,but in early 90's went on amiodarone and was great for 11 yrs. then it stopped working, as i got more paf's pac's and pvc's. had two ablations and now with the help of 160mg of sotalol i can again be very active. i still get pac's and occasional a fib but don't panic anymore. has anyone had complete success with TIKOSYN? is it a dangerous drug for someone that has slight structural damage to heart? would really appreciate a comment

I had ablation done in 1996 with Dr. Kidwell at CC for a troubling double bigeminy that terrorized me and I had become allergic to beta blockers.  I have Diabetes, had it then, it was not a comfortable procedure, it succeeded about 80 percent, they could not source the arrythmia long enough to burn the pathway completely, however, something worked because I have spells infrequently, and my pvcs don't really terrorize me anymore.
I am on Norpace only as needed, I get 30 capsules per year, and at the end of this year, I still had two left, so that was 28 times I needed meds in 365 days.  I was 52 years old and very overweight in 1996.  I still am fat, now 60, and I can walk and bike but I cannot drink coffee!  I had many spells going thru menopause and severe anemia made trouble for me, but in 2000 that ended because I had Y2K menopause!  Since that ended I feel much better.  Most of the time the spells I do have are  because of my asthma, my asthma meds and anxiety, I have a long history of panic attacks.
I am presently undergoing a cardiac work up and I am sure that my numbers will not be spectacular but as Elton John once sang, I'm still standing! I would hope that ablation has not done me any harm, but you never know.  But here is the thing, I had no quality of life before the ablation, there were good days, sporadically, now there are bad days, sporadically, and the majority of my time is spent managing my diabetes and my aging and my weight.  But I am free of the demon that my arrythmia had become.  Hope I have helped inform someone. Oh, and I have had
three very necessary surgeries since, knee, bladder, eye, and my first question to the doctor after is how was my heart?  And they have all told me that the heart skipped a couple of times and it was fine. Happy Easter and Passover!

JUST GOT MY 24 HR. HOLTER MONITER RESULTS BACK.SHOWED 1414 PVC'S.
4 WERE COUPLETS..83 PACS..6 ATRIAL COUPLETS..39 MINS. OF TACHYCARDIA. BIGEMINY,TRIGEMINY!!!!!! THIS SCARES ME TO DEATH.I WAS SO HOPING I COULD GET A PVC ABLATION. I HAVE UNIFOCAL PVC'S.BUT THE COMMENT FROM THE FIRST MESSAGE ABOUT A ABLATION POSSIBLY AWAKENING NEW ARRYTHMIAS SCARES ME TO DEATH. IM 31 AND ALREADY GOING THROUGH SO MUCH WITH THESE RYTHMS, I SURE DON'T WANT TO ADD ANY NEW ONES.BUT WHOSE TO SAY THESE WONT DETERIORATE ANYWAYS.MOMTO3 IS THE ONLY PERSON ON HERE THAT HAS MENTIONED HAVING A POSITIVE PVC ABLATION.DOES ANYONE HAVE ANY ADVICE ON IF I SHOULD TRY IT OR NOT.IM GREATLY BOTHERED BY PVCS EVERYDAY.MEDS ARE MINIMAL HELP AND I'LL HAVE TO TAKE THEM LONG TERM.AN ABLATION COULD BE A MIRACLE OR A DISASTER..ITS A HARD DECISION..P.S. DO COUPLETS POSE A GREATER RISK TO ME?

My husband takes Niaspan, but for lowering of his triglycerides.  It has some unpleasant side effects.  Sometimes he gets them, sometimes he doesn't.  Mostly having to do with flushing, tingling, and stomach cramps.  He really doesn't like to take this medication.  It is also recommended that if someone is using this medication, that they take it right before bedtime, and with food.  As I understand it, the manufacturer gets ALOT of calls concerning reactions to this med, though mostly harmless.  Hope this helps.

rw,

thanks for the post.

1) Is there anything that can help PVC's to stop hitting so hard and often?

Certainly no specific therapies. Alot of solutions are tied to trigger avoidance. Sometimes other specific drugs in the same class have less side effects in individuals, so another beta-blocker might help without tiring you out.

2) Would this long period have damaged my atrium enough to cause more chances for Tachycardias to take place?

There probably is some structural remodelling of your atrium from your valvular disease that places you at higher risk.

3) Do you think ablation for my PVC would cause others to start?

Unfortunately, some people undergoing ablation develop other arrythmias related to the procedure.

4) If I was your patient, what advice would you give me, besides see your doctor?

Eat healthy, maintain an active lifestyle, take you medications, wear your seatbelts.

good luck

Kim,

EF of 55% is normal. There is normally a 5% variant in interpreting the number.  Thus, your 55% could range from 50-60.

50% EF is low normal.  At your age and in good condition it should be around 60.  There are many factors that can affect EF so I think it is important for your Doctor to go over this with you and interpret it in view of everything else that is going on.  Arryhythmias can affect EF.

i noticed the doc hasnt answered this question yet maybe you can answer mine also....or someone else...
i got my echo results and my EF was 55%, i read this today:
"An EF between 40 and 55 indicates damage, perhaps from a previous heart attack, but it may not indicate heart failure."
could my ablation have caused damaged that in return made my ef decline? or possibly it could be from my frequent pvc's and sinus tach. i havent got to talk to a doctor about the results yet, only a nurse who told me normal was 40-50percent. i was like "ok cool mines above average" then i went on the net and figured out she had no clue what she was talking about. im only 18 years old i feel like mine should be higher than the low normal. i called medical records in cali today so i could get my ef from my echo done a few years back before the ablation, they couldnt give it to me unless i signed a release so its going to take awhile. i cant really say im surprised that its low normal because of the way ive felt since the ablation....
:(

Read this exciting news about a new drug that prevents sudden death:

http://news.bbc.co.uk/1/hi/health/3610701.stm

"A study on mice at the University of Columbia showed it completely prevented sudden death in animals with the same defect as people with heart failure."

It's exciting for those who have dangerous arrythmias and I wonder if it'll stop PVC's or not.
