I had a burch bladder repair done abdom. in dec 2006, the surgery did not work and my bladder fell 3 weeks later.  The doc told me i then needed a a&p repair along with a monarch tape sling.  I went threw with the surgery in mar 2007.  I have had nothing but problems ever since.  I ended up with a very large hematoma on the right side of my bladder, my bladder has been spasming ever since i left the hospital and there for i am still taking the meds to control the urgincy and leaking.  At my 6week post op i started bleeding alot and having big clots come out while urinating.  I was seen by a different doc that is in practice and he said he thought that i hade a hole in my bladder and that the tape was inside and referred me to a urologist for a scope.The urologist thinks that I could have a suture in my bladder from the first surgery because ofwhere the mass is located. He also said it is a possibility that it is a psuedo tumor caused by the surgery. I am having pain in my lower abd,and lots of swelling.  I am taking bentyl and prosed 4x daily. I dont know what to do or what either diagnosis means for me.  Can you explain what will happen if i have either one of these problems.  Also the urologist would like to biopsy the mass,he said he may have to remove part of the bladder and take out the sling. Please help im so confused.

This sounds like a complicated problem. A hematoma is another name for a blood collection which often clots after a period of time. The bladder spasms/urgency can occur of the bladder outlet is tightened too much, or if there is foreign material in the bladder. The late vaginal or urinary bleeding can happen if your tissue cannot heal around a foreign body (e.g., permanent suture or the sling tape). While it is possible that you could have a bladder tumor, I think the onset of symptoms is after your surgery, so surgery related complications have to be considered.  You will need a cystoscopy to evaluate your bladder to see if there are any foreign bodies (i.e., suture or sling tape) in the bladder, to explain your bleeding. Your doctor will also check a postvoid residual test to see if your bladder is emptying well to rule out a bladder outlet obstruction.  The necessary evaluation can be done by an appropriately trained urogynecologist or urologist. You should ask if your doctor has appropriate training beforehand....
