Blood Clot

A family member is having a clot in between the brain and the outer shell. Is it advisable to
go for a surgery. The person is getting severe headache. Whether it can be cured by medication.

You are not very specific.   A large blood clot in the brain can start shifting the brain sideways - and that is never good as it can cause permanent damage.   A good neurologist and / or neurosurgeon should be able to make the right decision, and you should trust them.
