Scrotum issue

My question to this forum is basically an issue ive ben having for quite sometime, until i got older i realized its just annoying now.  I first just want to say i was diagnosed with a variocele about a month ago which is fine, im doing fine i havent had any pain at all and it didnt require surgery.  But, i dont know if that helps or not. My scrotum sags about 2 inches from my penis tip which is i guess normal due to hotness and a relaxed state, but it never tightens up during an erection and or at ejaculation, ever. It only ever tightens up if im anxious, and really really cold.  Its never consistent i could blast the Air Conditioning and it would still sag very low, i dont understand the problem.  When i mean never, it never tightens during sex or at an arousal state and its really getting annoying, also the skin on the sac is very loose like strechy almost, is this normal? or a problem.

Difficult to say without evaluation.

Imaging the scrotum should be considered to rule out a mass, cyst, or fluid collection.  

Obtaining a urine test should also be done to evaluate for infection - although it would be an atypical presentation for this.

If the tests and imaging studies remain negative, it may be anatomic abnormality that is benign.

This question should be discussed with your personal urologist.
